The Tour de France scandal highlighted the need for an independent, nonjudicial international agency that would set unified standards for anti-doping work and coordinate the efforts of sports organizations and public authorities. The IOC took the initiative and convened the First World Conference on Doping in Sport in Lausanne in February 1999. Following the proposal of the Conference, the World Anti-Doping Agency (WADA) was established later in 1999. Over the past 150 years, no sport has had more high-profile doping allegations than cycling.16 However, few sports have been without athletes found to be doping.
Their social risks are managed by providing social support among the doping group who all share the same (secretive) use. Policy risks are reduced by anticipating anti-doping testing in order to circumvent a positive test. Similarly, economic risks, including loss of one’s livelihood, are managed by avoiding positive tests and ensuring no disqualification, loss of prize money, or loss of sponsorships. Similar systems have also been reported in competitive bodybuilding where coaches support competitors doping practices through advising on na vs aa what to take, how to acquire substances, proper dosing, and managing risks (Andreasson & Johansson, 2020; Monaghan, 2001).
Anabolic steroids are usually taken either in tablet form or injected into muscles. Alcohol use disorder (AUD) is the medical diagnosis for prolonged and severe drinking that is causing problems in a person’s life. It is extremely prevalent and a serious medical issue that can have fatal consequences. On 22 October 2012 Lance Armstrong was officially stripped of his Tour de France titles since 1 August 1998.[187] As a response to the decisions of the USADA and UCI, Armstrong resigned from the Lance Armstrong Foundation.[188] He later admitted to doping in an interview with Oprah Winfrey. Previously there was no testing between 11pm and 6am, providing a potential window of opportunity for micro-dosing products, such as EPO, without being caught.
Materials and methods
WADA is the international independent agency that publishes the World Anti-Doping Code, which is the document harmonizing anti-doping policies in all sports and all countries.61 The Code was first adopted in 2003 and became effective in 2004. Quite a few drugs, including steroids and growth hormone, that are regularly banned by sports organizations are useful medical treatments, especially for sports injury recovery. Erythropoietin (EPO) is largely taken by endurance athletes who seek a higher level of red blood cells, which leads to more oxygenated blood, and a higher VO2 max. An athlete’s VO2 max is highly correlated with success within endurance sports such as swimming, long-distance running, cycling, rowing, and cross-country skiing.
It is an electronic document about an athlete that contains certain markers from throughout their career. If these change dramatically, it alerts officials that the athlete might be doping. Blood testing is capable of detecting EPO and synthetic oxygen carriers, but not blood transfusions. Narcotic analgesics and cannabinoids are used to mask the pain caused by injury or fatigue – but in practice can make injuries worse.
The most commonly used substances are androgenic agents such as anabolic steroids. These allow athletes to train harder, recover more quickly and build more muscle, but they can lead to kidney damage and increased aggression. In 2007, the NFL announced stricter anti-doping policies, increasing the number of players tested and the number of random off-season tests; four years later, the NFL became the first major American sports league to approve blood testing for HGH, a policy which was finally implemented in 2014. In countries where the use of these drugs is controlled, there is often a black market trade of smuggled or counterfeit drugs.

Opponents claim that with doping legal, all competitive athletes would be compelled to use drugs, and the net effect would be a level playing field but with widespread health consequences. A common rebuttal to this argument asserts that anti-doping efforts have been largely ineffective due to both testing limitations and lack of enforcement, and so sanctioned steroid use would not be markedly different from the situation already in existence. Currently modafinil is being used throughout the sporting world, with many high-profile cases attracting press coverage as prominent United States athletes have failed tests for this substance. Some athletes who were found to have used modafinil protested as the drug was not on the prohibited list at the time of their offence, however, the World Anti-Doping Agency (WADA) maintains it is a substance related to those already banned, so the decisions stand. Modafinil was added to the list of prohibited substances on 3 August 2004, ten days before the start of the 2004 Summer Olympics.

Taking these into account, we next apply the risk environment framework to the current context to analyse how anti-doping policies have created an intolerant environment that drives doping into the shadows of sport, increasing various risks to athletes along the way. Following this, we examine how groups have organized systems of doping using cymbalta alcohol strategies, similar to those outlined above, that work to reduce harm to athletes and enable safer use of PEDs. These models each offer benefits to athlete health, though they leave open many issues of implementation within the wider sport environment that has been saturated with anti-doping narratives of drug-free sport and zero tolerance for doping. The prevalence of doping is unclear, though there is a significant gap between prevalence estimated by researchers and the official testing results. It is difficult to determine the effect anti-doping policies have had on doping prevalence, as there were no baseline prevalence studies before WADA’s founding. However, the discrepancy between the positive test rate and even the lowest prevalence estimates indicate that a large number of athletes who report engaging in doping are not being detected, potentially exacerbating PED-driven inequalities between athletes. In this way, anti-doping policies may mirror the limited effectiveness of recreational drug prohibition in deterring consumption or punishing violations, lessons best illustrated in the case of the United States.
Even athletes who were not part of the doping system suffered reputational and economic damages. Many of those who were implicated received competition bans up to four years. Even if a PED is originally used for injury or surgery recovery, the drugs’ effects can be addictive and lead to more long-term use and unfair advantage in competition. While PEDs may seem like the quick ticket to athletic glory, they are deadly.
It is unclear what will be the final outcome of doping war, but new questions and issues constantly present new challenges for both groups. For example, how each side will respond and adjust when unexpected outside forces – such as the current Covid-19 pandemic that has led to the postponement of World and Olympic level events – upset the tug of war. Future research on anti-doping policy and harm reduction may look more closely at the ways known doping systems have developed and their strategies for reducing various risk factors in order to enable doping. By considering these systems, sport researchers and policymakers may find new ways to incorporate harm reducing strategies to produce a less risky sport environment. It is possible, though, that the most effective doping systems for reducing harms may be the ones that have thus far avoided detection.

Former Olympic 1500m champion Lord Coe was named the new president of the IAAF on Wednesday and says he is determined to prove athletics’ world governing body is committed to ridding the sport of drug cheats. Some scientists have questioned the passport’s efficiency – especially when complicating factors such as training at altitude are factored in – but also its sensitivity to micro-dosing, a little-but-often approach to doping. Brought in by Wada in 2009, the passport aims to reveal the effects of doping rather than detect the substance or method itself. Some by-products of doping substances are so small they may not produce a strong enough signal for detection.
